Appointments are made for our surgeons from our main rooms in Donvale. Please contact our team to arrange a suitable appointment time on 03 9988 2888 or click here to email. You will be provided with patient information at the time of booking your appointment.
Please ensure that you bring along the following:
- GP referral
- Results of any testing done prior to your appointment (xrays, blood test, etc.). If you have had xrays or scans (MRI, CT etc.) and do not have the hard copies of the films, we can mostly view these images and the reporting through the radiology provider who performed the tests. In this case ensure you know the name and location of the radiology group where you had the scans
- Completed registration form – Click the link below to complete and submit your registration form online, or you can complete one at the time of your appointment.
- Names of any other specialists or allied health professionals (physio, lymphoedema therapist, heart specialist etc.) who is involved in your care
- Current medication list
- Workcover or TAC claim information (if applicable) including Employer and Workcover Agent, Claim Number and Date of injury
